Draft a Treatment Plan Note From a Dictation
ScreenApp is not HIPAA compliant and does not sign a Business Associate Agreement (BAA). Do not upload protected health information (PHI). Use it to dictate a de-identified case, a training scenario, or a mock assessment, and get an AI-drafted treatment-plan note back: presenting problem, goals, interventions, and next steps, written as text for you to edit.
This is a drafting step, not a record system. Describe a case out loud without a name, date of birth, medical record number, or anything else that identifies a real patient, or work from an invented or supervision case instead. The AI turns your dictation into a first draft. Rewrite it in your own words before anything from it goes into a real chart.
What you get:
- A transcript of your dictation or uploaded recording, in 100+ languages
- An AI-drafted note structured as goals, interventions, and next steps, based on what you say
- custom vocabulary for transcription, so a drug name or clinical term is transcribed correctly
- Export as PDF, DOCX, TXT, SRT and VTT
- Free plan: 2 transcriptions of recordings up to 45 minutes each, with AI chat
How to Draft a Treatment Plan Note
- Dictate or upload a de-identified summary: Record yourself describing the case, with no names or identifying details, through your browser or the ScreenApp mobile app, or upload an audio or video file you already have.
- Ask the AI to structure the note: Once the transcript is ready, open AI chat and ask it to draft the note as a treatment plan: presenting problem, goals, interventions, and next steps. AI templates and documents follow the same structure.
- Edit before you use it: Read the draft against your dictation, correct anything the AI misread or invented, and rewrite it in your own words before it goes anywhere near a real patient record.
Accuracy depends on the audio. A quiet room and a phone or headset mic close to the speaker gives the cleanest transcript. See how we measure accuracy.

- vs Freed: Freed is built to be HIPAA compliant and signs a BAA on its Group plan, drafting notes directly from live clinical sessions. ScreenApp is not HIPAA compliant and is built for de-identified dictation and practice drafts, not real patient sessions.
- vs Mentalyc: Mentalyc states HIPAA and PHIPA compliance and integrates with EHR and telehealth platforms for therapists documenting real sessions. ScreenApp encrypts recordings with AES-256 and publishes its controls on the trust center, but it is not HIPAA compliant and has no EHR integration, so it is not a substitute for Mentalyc’s clinical workflow.
Who Drafts Notes With ScreenApp
Clinical supervisors and training programs dictate an invented or de-identified case to produce an example treatment plan for teaching, then discuss where the AI draft is right, wrong, or too generic.
Therapists and counselors in training practice the format of a treatment plan on a mock case before writing the real note by hand in their own EHR, without ever putting a real client’s details into ScreenApp.
Nursing, OT, and PT students dictate a case study assignment and get a structured first draft of goals and interventions to critique and rewrite, as a way to learn the format faster than starting from a blank page.
Solo practitioners draft a personal note template or a practice script from a hypothetical case, then adapt the structure to their own documentation system.
Case managers and care coordinators outline a generic care-coordination structure for training materials or internal process documents, not for an individual’s real record.
